Do you accept health insurance for services?
At FUNdamentals, services will not be submitted through health insurance companies, nor will subsidies be accepted.

If requested, we can provide a Superbill for services. This allows caregivers the option to independently submit to their health insurance company for possible reimbursement, if they so choose.

We are also happy to accept FSA (Flexible Spending Account) and HSA (Health Savings Account) cards as payment.
Please note:
-
FUNdamentals receives payment at the time of purchase.
-
Eligibility of services is determined by your plan administrator or insurance provider, not by FUNdamentals.
-
If your provider later deems a purchase ineligible, you may be responsible for repayment to your FSA/HSA provider.
-
Itemized receipts are available upon request for your records or reimbursement needs.
