Service Description
OT helps individuals develop skills that improve their daily life. Sessions focus on how children move, play, and communicate with the world around them. Fine & Gross Motor skills / Self-Regulation / Sensory Processing Complete the booking process online. Once we review your request, a team member will contact you directly to schedule your session. Availability - 45 min sessions between the hours of: Tuesdays 12:00-2:00 Thursdays 11:00-2:00 *Sessions are privately paid. We do not accept insurance. **Payment in full must be receive before attending your session. Requirements: 1) Signed waiver for all attendees 2) Sessions must be booked 24 hrs in advance 3) Socks are required for children and adults
Cancellation Policy
REGISTRATION: Registration 24 hours in advance is required to ensure that our staff are fully prepared for the number of participants for each session. If you have any trouble booking online, please do not hesitate to give us a call: 401-206-4030. PAYMENT: Payment in full will be required online at the time of registration in order to secure your child’s enrollment. LATE PICK UP FEES: FUNdamentals reserves the right to charge a fee for late pick up. If a child is picked up late, the parent/guardian will be subject to an immediate charge of $5 per child. An additional $10 will be charged for every 15 minutes after the pick up time. If the parent/guardian is unable to pay the late fee at the time of pick up. A request for payment will be sent. Repeat late pick up will result in a child’s place being canceled without a refund for any remaining services. LATE PAYMENTS: If fees are not received within 7 days of payment a fee of $10 will be added and an additional request will be sent. If fees are not paid 14 days from the original request for payment, you forfeit any scheduled sessions and no refunds or credits of previous payments will be offered. Should you fail to make payment after a service has been provided, you will be sent two payment reminders. Failure to make payment after this will result in the case being sent to a debt collection agency on our behalf and further charges will be added. ABSENCES & ILLNESS: No refunds or exchange of services will be offered if a child is sick and misses a session due to illness. If a child isn’t well enough to attend a session, we ask that they be kept home. If a child becomes ill during a session, we reserve the right to contact the parent/guardian and request immediate pickup. CANCELLATIONS & FACILITY CLOSURES: FUNdamentals reserves the right to cancel a session at any time due to unexpected circumstances. Small group sessions require a minimum of three attendees in order to run the group. The decision to cancel a group will be made at least three business days prior to the start date. If a class is canceled by FUNdamentals, a full refund will be given, or a second time and date will be offered. REFUND POLICY: Sessions purchased are non-refundable unless canceled by FUNdamentals, in which a full refund will be provided.
